DR. CHERUP’S OPINION ON MINI-TUMMY TUCKS
Mini-Tummy Tucks are useful only in nulliparous (no pregnancies) or one-pregnancy patients. I also use them on men when I am doing a ventral hernia repair and a minimal skin resection is possible. Patients that have a low-lying umbilicus – at their waist or lower- are not candidates, for, in the end, the position of their umbilicus would be aesthetically too low, and unnatural.
In a Mini-Tummy Tuck, the lower skin flap is undermined up to the umbilicus. The surgeon then must choose whether to ignore the upper abdomen, treat it with liposuction, or undermine the skin and repair the muscles. This, of course, depends on what is “wrong”. The umbilicus can be “floated”- detached from the muscle of the abdominal wall and keep attached to the skin flap. This maneuver allows further upward dissection and treatment. After that work is done, the skin is re-draped and 4-6 centimeters of lower abdominal skin is resected and closed. I always use drains in every TT, even though I also always use plication sutures to anchor the deep fat to the abdominal wall. The sutures cut down on the JP drain amounts, and shorten the healing time.
Mini-Tummy Tucks are NOT indicated in the repair of 2+ pregnancies patients, weight-loss patients, or panniculectomies. I am always unimpressed by my Mini-TT, and often wish I had “talked the patient into” a full abdominoplasty, where the results are always “WOW!”. I charge only 10% less for a Mini- TTs, because there is more decision-making in the process, and patients are sometimes less than thrilled, and then I have to deal with a less-than-satisfied patient.
